If you are using insurance, you will likely need a prescription for physical therapy. Please have your referring physician fax to: 808-840-0044

Please check with your insurance company for individual specifications, as each plan is different in regards to copays and deductibles.

CANCELATION POLICY:

Please contact us within 24 hours of your scheduled appointment for any cancellations. You will be charged in full ($125) for cancellations with less than 24 hours notice.
